چکیده

A62-year-old man presented to our emergency room with severe retrosternal pain. An ECG showed an acute lateral myocardial infarction, and the patient was referred for cardiac catheterization. One and a half hours after the onset of acute chest pain, coronary angiography revealed a thrombotic occlusion of the intermedial branch of the left coronary artery. Mechanical recanalization of the infarct-related artery was performed, and the underlying stenosis was treated with stent implantation. Immediately after the procedure, the patient was free of pain, and the electrocardiographic changes improved. The next day, the patient complained of short episodes of chest pain without any changes in the ECG or biochemical cardiac markers. A contrast-enhanced chest CT was performed using a multislice CT scanner (Somatom VolumeZoom, Siemens) 24 hours after initial hospital admission. ECG-triggered arterial phase images focused on the ascending aorta (not shown) ruled out an aortic dissection. Conventionally acquired axial venous phase images (Figure, A and B) and sagittal-oblique reconstructions (Figure, C) of the entire thorax revealed a hypodense area within the posterolateral wall and both papillary muscles of the left ventricle. The non–contrast-enhancing myocardial areas were attributed to the subacute infarcted zone, and an echocardiogram 1 week later confirmed the diagnosis by showing morphological changes and hypokinesia of the posterolateral wall and of both papillary muscles.

Acute myocardial ischemia after aortic valve replacement: a comprehensive diagnostic evaluation using dynamic multislice spiral computed tomography.

We describe the case of a 72-year-old man presenting with endocarditis and clinical signs of acute myocardial ischemia after biological aortic valve replacement. A comprehensive cardiac dynamic multislice spiral computed tomography demonstrated: (1) an endocarditic vegetation of the aortic valve; (2) a subvalvular leakage feeding a paravalvular pseudoaneurysm based on an aortic root abscess wit...

Noninvasive Assessment of In-Stent Restenosis of the Coronary Artery with Using 16-Slice Computed Tomography

tion has been widely used for managing coronary artery occlusive disease. However, the risk of in-stent restenosis is about 10-40%, as determined on the angiographic follow-up at 6 months, and this remains a clinical issue in cardiology (1-3). Early identification of in-stent restenosis is important to reduce the number of recurrent ischemic episodes and to prevent myocardial infarction, thereb...

Evaluation of plaque texture by means of multislice computed tomography in patients with acute coronary syndrome and stable angina.

BACKGROUND In the present study, multislice spiral computed tomography (MSCT), which allows non-invasive assessment of coronary artery plaque, was used to compare the CT density of plaque between patients with acute coronary syndrome (acs) and those with stable angina (sa). METHODS AND RESULTS MSCT was performed in 20 patients with ACS (17 with acute myocardial infarction, 3 with unstable ang...

Assessment of myocardial viability in reperfused acute myocardial infarction using 16-slice computed tomography in comparison to magnetic resonance imaging.

OBJECTIVES The aim of this study was to examine if contrast-enhanced multislice spiral computed tomography (MSCT) is comparable to contrast-enhanced magnetic resonance imaging (MRI) for depiction of acute myocardial infarction (MI). BACKGROUND Delayed-enhancement MRI of MI is well established, but there are no clinical reports about MSCT for this indication. Early perfusion deficit on MSCT ha...
